There may be medical or biological issues, such as Rage Syndrome, that
are well beyond the scope of a trainer. Some problems are idiopathic and
cannot be pinned down. There can be neurological disorders, thyroid
dysfunction, etc that need veterinary care.
A good dog trainer knows his limitations and will tell you that a different
road needs to be taken. A proud, know it all dog trainer can cause more
harm than good.
